Advance Care Planning: Make Your Wishes Known

If something happened to you and you weren’t able to speak and make your own medical decisions, do you have a plan in place? Attend this session and learn why it’s so important to have an advance directive for healthcare. You will leave the session with the tools and documents to create your own advance directive for healthcare.

This workshop will encourage participants to consider their wishes for their medical care in the future, in emergency situations, and for end-of-life care; and ensure those wishes are legally documented. It can be difficult to think about medical emergencies and illness. Learn how to start the conversation. By planning ahead, you can be empowered to ensure your wishes are heard and followed.
This is a hybrid program offered virtually on Zoom and in-person at 215 Revere Drive, Northbrook, IL 60062.
